| BSA
Annual Health and Medical Record. Parts A and B
must be filled out by families every year and
given to the troop so that your son may camp
with the troop and participate in troop
activities. Part C must be completed
by a medical doctor who examines your son and is
required for any Scout who attends a BSA summer
camp such as Camp Tuscarora, Camp Durant or Camp
Raven Knob. The form can be completed
on-line, printed, and returned to Troop 451. |
BSA
Youth Protection Training is required for
any Adult who wants to participate in Troop
activities. You must complete your
training and forward notivication and your
certificate to Troop Committee Chairman Brendan
Turner.

Activity Consent Form. This form is
recommended for unit use to obtain approval and
consent for Tiger Cubs, Cub Scouts, Webelos
Scouts, Boy Scouts, Varsity Scouts, Venturers,
and guests (if applicable) under 21 years of age
to participate in a den, pack, team, troop, or
crew trip, expedition, or activity. This form is
required for use with flying plans and should be
attached to the flying plan application. It is
recommended that parents keep a copy of the form
and contact the tour leader in the event of any
questions or in case emergency contact is needed
